What you Should Bring:

  • State Issued Driver’s License or Identification Card

  • Secondary ID Card, such as Social Security Card

  • Health Insurance Card(s)

  • If you are visiting our office for the first time, we recommend you to print these forms, fill them out and bring them to your first visit.This will save your time waiting at the office.


Insurance & Payment:

We participate with many insurance companies and we bill for most of our services. You are responsible for your deductibles.

 If your insurance requires a co-pay, you will be expected to pay at the time of your visit. A billing fee will be added to co-pays not paid at the time of service.

Prescriptions:

Prescriptions for routine medications should be filled during your regularly scheduled appointments. Please remember to bring your medication list or medications with you to your appointment.


Our office requires a 24 hour notice for all appointment cancellations and re-schedules.

When you miss a scheduled appointment without calling to cancel or re-schedule, it prevents other patients from receiving medical care. Please call our office if you are unable to keep your appointment. There will be a fee of $50.00 for any missed appointment.
